Inpatient care Quality Improvement Project.

 

The Inpatient care quality improvement cycle started with the goal of improving patient care by identifying lapses, and checking progress in 4 domains of clinical practice that impact the quality of inpatient care namely; 

 

·      History taking 

·      Clinical/ physical examination

·      Lab and imaging data analysis

·      Full case analysis

 

Using the online learning portfolios of the students of 2016 batch; which was divided into 4 quarters; these 4 domains were graded based on performance from 0-4. I collected this raw data and processed the data to represent it statistically (quarter mean and cumulative mean scores). Thus enabling us to check for progress/ changes in specific domains and overall clinical practice.

 After processing the data, I systematically entered the data into an excel spreadsheet and made relevant tables/ graphs to represent our findings. 



These online learning portfolios were selected for cycle 3. These blogs were graded as done in previous cycles by Dr. K. Vaishnavi PGY3 GM and Dr. Aditya Samitinjai SR GM. 
I then collected, analyzed and presented the scores as relevant summarized graphs and charts.

Summarized Data:-

QI Cycle 1 and Cycle 2 of this project were already run and published in March

2022 and April 2022 respectively. QI Cycle 1 included only 7 online learning portfolios, 1 from 2017, 1 from 2018 (due to unavailability of data) and 5 online learning portfolios from 2020. QI Cycle 2 included a total of 58 online learning portfolios from May 2020 to June 2021.

 

Since the beginning of Cycle 3 of our Quality Improvement programme, we have blogged 47 cases directly or indirectly related to rheumatology over the time period of April 2021 - May 2022, the full duration of the intern year of our 2016 MBBS batch. This also includes the 3 online learning portfolios included in QI cycle 2 to be all inclusive of the performance of this batch of interns.

 

Data will be presented in the same way presented in Ql cycle 1 and cycle 2, although explanation for scoring will be omitted due to the large number of online learning portfolios involved. For more info on how we score our online learning portfolios, please see the link to cycle 1 at the top of this post.

 

The total number of online learning portfolios included in QI cycle 2 was 47, with 20 (42.5%) from 2021 and 27 (57.4%) from 2022. 

 

Online learning portfolios included in QI Cycle 3, sorted by month and year. These were grouped as follows -

 

1. April 2021 - June 2022 - Quarter 1

2. August 2021 - October 2022 - Quarter 2

3. November 2021 - January 2022 - Quarter 3

4. February 2022 - May 2022 - Quarter 4

 

In the data presented below, bars shaded in orange are data from 2021 and those shaded in green are from 2022. 

 

The average total scores of online learning portfolios (from a total of 16 points) sorted by quarters as listed above. There is an improvement of the average total scores of the blogs starting from Quarter 2 through Quarter 4. The average total score of blogs from Quarter 1 being 10.9 is attributed to the overlap between the old ( 2015 ) and new ( 2016 ) intern batches. Overall, there is an increase in the cumulative average scores of the blogs from QI Cycle 3 compared to Cycles 1 & 2.

 

Q1 Cycle 1 = 10.3/16

Q1 Cycle 2 = 10.9/16

Q1 Cycle 3 = 12.2/16

 

The average scores in the domain of history taking are presented above, sorted by quarters. The cumulative average across all 4 quarters was 3.2 and 3 out of 4 quarters were above the cumulative average. We have managed to achieved our initial target average of 3 out of 4 points this Cycle, exceeding by 0.2 points.

 

QI Cycle 1 = 2.4/4

Q1 Cycle 2 = 2.6/4

Q1 Cycle 3 = 3.2/4

 

The average scores in the domain of clinical or physical examination are presented above, sorted by quarters. The cumulative average across all 4 quarters was 2.8 and 2 out of 4 quarters were at or above the cumulative average. However, we set out with an initial target average of 3 and fell short by 0.2 points. Quarter 2 was our most successful with an average score of 2.8.

Q1 Cycle 2 = 2.8/4

Q1 Cycle 3 = 2.8/4

 

The average scores in the domain of lab & imaging data analysis are presented above, sorted by quarters. The cumulative average across all 4 quarters was 3.0 and 3 out of 4 quarters were at or above the cumulative average. We managed to achieve our initial target average of 3 in this Cycle. Quarters 2 & 3 were our most successful with an average score of 3.1.

 

QI Cycle 1 = 2.6/4

Q1 Cycle 2 = 2.9/4

Q1 Cycle 3 = 3.0/4

 

The average scores in the domain of full case analysis are presented above, sorted by quarters. The cumulative average across all 4 quarters was 3.2 and 3 out of 4 quarters were at or above the cumulative average. We have managed to achieve our initial target average of 2.8 in this Cycle. Quarters 2 & 3 were our most successful with an average score of 3.3, Quarter 4 falling short by just 0.1.
